Pursuant to the Arkansas
\nFreedom of Information Act (Ark. Code Ann. \u00c2\u00a7 25-19-101), I hereby request
\ncopies of the public records identified below. I am requesting copies of the
\nfollowing documents:<\/font><\/p>\n

(a)                
\n<\/span><\/font><\/span><\/span>all documents submitted to any agency or
\ndivision of the United States Government seeking reimbursement made for
\npayments under the State Children’s Health Insurance Program
\n(“SCHIP”) program which detail any of the following information: (i)
\nthe full amount of reimbursement sought; (ii) the full amount of payments made
\nby the State of Arkansas or any agency thereof under the State Children’s
\nHealth Insurance Program (“SCHIP”) Program; (iii) any demographical
\ninformation concerning recipients of benefits under the State Children’s Health
\nInsurance Program (“SCHIP”) Program; and (iv) any information
\nspecifying the number of recipients (including pregnant women) receiving
\nbenefits under the State Children’s Health Insurance Program
\n(“SCHIP”) program that are not lawfully present in the State of
\nArkansas or United States of America.<\/o:p><\/span><\/font><\/p>\n

(b)                
\n<\/span><\/font><\/span><\/span>all
\ndocuments containing the following information regarding the State Children’s
\nHealth <\/font><\/span>Insurance Program (“SCHIP”) program: (i) the full
\namount of reimbursement sought; (ii) the full amount of payments made by the
\nState of Arkansas or any agency thereof under the State Children’s Health
\nInsurance Program (“SCHIP”) Program; (iii) any demographical
\ninformation concerning recipients of benefits under the State Children’s Health
\nInsurance Program (“SCHIP”) Program; and (iv) any information specifying
\nthe number of recipients (including pregnant women) receiving benefits under
\nthe State Children’s Health Insurance Program (“SCHIP”) program that
\nare not lawfully present in the State of Arkansas or United States of America.<\/o:p><\/span><\/font><\/p>\n

(c)                
\n<\/span><\/font><\/span><\/span>all
\ndocuments identifying any amount spent by the Department of Human Services of
\nthe <\/font><\/span>State of Arkansas under the State Children’s Health Insurance
\nProgram (“SCHIP”) Program or any other program which may provide a
\nbenefit to any person who is not lawfully present in the United States or State
\nof Arkansas (including pregnant women);<\/o:p><\/span><\/font><\/p>\n

(d)                                
\n<\/span><\/font><\/span><\/span>all documents identifying the number of anchor babies on the State
\nChildren’s Health <\/font><\/span>Insurance
\nProgram (“SCHIP”) program at any time in the fiscal year ended June
\n30, <\/font><\/span>2007;<\/o:p><\/span><\/font><\/span><\/p>\n

(e)                                 
\n<\/span><\/font><\/span><\/span>all documents identifying any programs available that will or may
\nprovide benefits for <\/font><\/span>any
\nservices or other monetary payment without regard to whether the recipient <\/font><\/span>(including pregnant women
\nreceiving prenatal services) is lawfully inside the State of <\/font><\/span>Arkansas or United States; and<\/o:p><\/span><\/font><\/span><\/p>\n

(f)               
\n<\/span><\/font><\/span><\/span>all documents identifying the number of recipients of any program of or
\noverseen by the <\/font><\/span>Department
\nof Human Services without regard to whether the recipient (including <\/font><\/span>pregnant women receiving
\nprenatal services) is lawfully inside the State of Arkansas or <\/font><\/span>United States.<\/o:p><\/span><\/font><\/span><\/p>\n

Please be advised that I am
\nspecifically excluding from this request any documents that are <\/font><\/span>protected from disclosure by the
\nFamily Educational Rights and Privacy Act of 1974,20 USC \u00c2\u00a7 1232g.<\/font><\/span><\/p>\n

The requested information may be
\nprovided to me at the address specified above. I offer to pay <\/font><\/span>the actual cost of reproduction,
\nas defined by statute, either in advance of copying or pursuant to a <\/font><\/span>statement or invoice from you after copying occurs.<\/font><\/span><\/p>\n
